The NCVEC Form 605 should not be confused with the FCC Form 605. The FCC will not accept the NCVEC Form 605. http://www.arrl.org/605-instructions

Testing is on the first Saturday of each month except December. Testing begins 9:00 AM and runs to 11:00 AM. Walk-ins are welcome.

Bring a photo I.D., your original and photocopy of your Ham license, a check or exact change of $15.00 made out to ARRL/VEC, and the completed NCVEC Form 605.
An FRN or FCC Registration Number is used in place of your Social Security Number, for all transactions with the FCC. Apply for it prior to first-time testing: https://apps.fcc.gov/cores/userLogin.do
There is a new form for each tester to fill out, answering the question concerning conviction of a felony. Do not confuse this with the FCC Form 605, as they are separate documents.
